Report an Issue
This form provides chauffeurs and team members with a quick and easy way to report any issues that occur during their shift — including vehicle, navigation, app, or client-related concerns — as they happen.
While some items may also be captured later in your Post-Reservation Feedback Survey (PRFS), that survey is completed after the job is finished. This form is designed to make reporting faster and more immediate — especially if something comes up mid-trip, between runs, or during downtime.
Submitting this form helps us resolve problems sooner, identify recurring issues, and maintain our high standards of safety, reliability, and client service.
